PREMIER PROGRAMMING
Transformation Health PRP has a full range of on-site and off-site services that will address the following areas and more:

An increase of supports through recreational activities and group sessions

Connection to community resources/Assistance with learning transportation skills and community navigation

Navigation of Social Service systems (i.e., SSA, DSS, local housing agencies) Community Advocacy/referrals

Life skills training (i.e., grooming, hygiene, nutrition, dietary planning, budgeting/money management, self-administration of medication) Improving crisis management, self-management, and recovery skills

Assistance with scheduling and maintaining appointments
​

Coordination of care with other providers/treatment team members (i.e., therapist, psychiatrists, PCP, substance abuse providers, specialists)
WHO IS ABLE TO RECEIVE PRP
Prospective clients Must meet the following criteria:

Have a mental health diagnosis that meets the priority population definition as determined by the Mental Health Authority.

Enrolled or eligible for Maryland ​Medical Assistance Program.
