If you have not registered your client-patient portal, please check the e-mail you provided at the time of your referral onboarding and use the link provided to register for your client-patient portal. If you can not locate the e-mail sent to you during your intake process, please contact the main office for assistance.
IMPORTANT INFO: EACH CLIENT MUST HAVE A SEPARATE E-MAIL ADDRESS FOR EACH INDIVIDUAL CLIENT IN THE HOUSEHOLD. TO CREATE A CLIENT-PATIENT PORTAL. PLEASE CREATE A SEPARATE E-MAIL PER FAMILY MEMBER.
