Psychiatric Rehabilitation Programs
ADULT PSYCHIATRIC REHABILITATION PROGRAM (PRP)
Transformation Health Psychiatric Rehabilitative Program for Adults provides quality rehabilitative and support services to adults 18 and older who have serious or persistent mental illness and are referred by a licensed therapist. This program also focuses on goals created by the client and is implemented through Individualized rehabilitation with the support of an assigned Rehabilitation Specialist.

Transformation Health ADULT PRP has a full range of on-site and off-site services addresses the following areas and more:

An increase in support through recreational activities and group sessions ​

Connection to community resources/Assistance with learning transportation skills and community navigation

Navigation of Social Service systems (i.e., SSA, DSS, local housing agencies) Community Advocacy/referrals ​

Life skills training (i.e., grooming, hygiene, nutrition, dietary planning, budgeting/money management, self-administration of medication) Improving crisis management, self-management, and recovery skills
